This legislation significantly enhances access to diabetes outpatient self-management training services (DSMT) for Medicare beneficiaries. It expands the covered hours to include an initial 10 hours of individual or group educational services, available until used, followed by an additional 2 hours annually. Crucially, the bill removes previous limitations on the quantity or duration of these services when deemed medically necessary by a physician or qualified non-physician practitioner, who can now also make referrals. To further improve access, the bill eliminates beneficiary cost-sharing for DSMT services, ensuring Medicare covers 100 percent of the cost and waiving the deductible. Additionally, it mandates the Center for Medicare and Medicaid Innovation (CMMI) to establish a model by January 1, 2026, to test the provision of virtual diabetes outpatient self-management training services . This CMMI model will evaluate the impact of virtual services on improving health outcomes, such as reducing A1c levels and hospitalizations, and on increasing utilization in underserved communities while lowering overall Medicare expenditures.
