This bill, titled the "Coordinating Care for Senior Veterans and Wounded Warriors Act," requires the Secretary of Veterans Affairs, in consultation with the Secretary of Health and Human Services, to implement a pilot program. This program is designed to coordinate, navigate, and manage care and benefits for covered veterans , defined as those enrolled in both the Medicare program and the VA's annual patient enrollment system. The primary purposes are to improve access to healthcare services, enhance care outcomes and quality, lower costs, and eliminate gaps or duplication of services for these veterans. The pilot program will be administered through the VA's Center for Innovation for Care and Payment and will operate in not less than three but not more than five Veterans Integrated Service Networks, specifically including rural and medically underserved areas. A key provision is the assignment of a case manager to each participating veteran, responsible for developing an individualized needs assessment and care coordination plan, and assisting with navigating both VA and Medicare systems. The Secretary is also directed to utilize existing value-based care models and, to the greatest extent practicable, contract with private sector entities for program design, implementation, and management, including care coordination. The pilot program is set to run for a three-year period, during which the Secretary must track various metrics such as veteran participation, reliance on VA and Medicare services, quality, cost, access, and patient and provider satisfaction. Regular quarterly reports on the program's development, implementation, and results are required for Congress, culminating in a final report that includes a recommendation on whether to extend or make the program permanent.
