Transition to Independence Act of 2018 This bill directs the Centers for Medicare & Medicaid Services to establish a demonstration program under which a state that has a Medicaid Buy-In program and meets other specified requirements may receive bonus payments for expanding employment opportunities for working-age individuals with disabilities who receive Medicaid-funded home- or community-based services.
